Get the Life You Want: A Transition Toolkit
Making a Transition Toolbox helps self-advocates, teachers, support people, and families to plan for the transitions people with disabilities make as they move into adulthood during high school. This is not a teaching curriculum. It is a way to explore and gather the information you might need after graduation and put it in one place.
The toolbox has checklists on topics that a student may want to talk about during their transition meetings. The topics range from voting to relationships and job-searching. Each topic includes a story written by a disabled adult giving transition advice. Each topic provides questions a student may want to discuss before, during or after a transition meeting. The checklists suggest information to look for before you graduate!
